NPS maintains institutional licenses for approved survey/questionnaire tools to help protect sensitive information and ensure compliance with human subjects research requirements. If your work involves research with human participants, you must also consult the Human Research Protection Program Office and obtain approval from the Institutional Review Board (IRB) prior to data collection.

Human subjects research
Research involving human participants must use institutionally approved tools that meet data protection and compliance requirements. At NPS, this includes Qualtrics, which is licensed and supported for human subjects research.

Non-research surveys and questionnaires
For activities that do not constitute human subjects research, a variety of tools may be appropriate depending on the purpose and data sensitivity. Microsoft Forms is one commonly used option available through NPS.
Access Microsoft Forms:
Go to office.com > sign in > select the Forms app.
